Sports Transportation Forms and Volunteer Driver Information
SHHS thanks our volunteer drivers who transport our student athletes to sporting events. If you wish to become a volunteer driver, please see the information below.
SHUSD Volunteer Drivers: can drive any SHHS student. To be an approved volunteer driver, you must have on file with SHHS:
- Volunteer Application Packet - click here or download forms
- Background check - please contact Diana Cruz for the screening application
- TB negative test result
- Completion of Mandated Training – please contact call Diana Cruz for information
- Completed Volunteer Auto Use Form
- Copy of valid driver’s license
- Copy of current auto insurance policy declarations (minimum of $100K/$300K liability)

SHHS agradece a nuestros conductores voluntarios que transportan a nuestros estudiantes atletas a los eventos deportivos. Si desea convertirse en un conductor voluntario, consulte la información a continuación.
Conductores Voluntarios de SHUSD: pueden conducir a cualquier estudiante de SHHS. Para ser un conductor voluntario aprobado, debe tener en los archivos de SHHS:
- Paquete de Solicitud de Voluntariado: haga clic acqui o descargue los formularios
- Verificación de Antecedentes – contacte a Diana Cruz para la solicitud de verificación de antecedentes
- Resultado negativo de la prueba de tuberculosis
- Finalizacion de Entrenamiento Obligatorio – contacte a Diana Cruz para información
- Copia de licencia de conducir
- Copia de las declaraciones actuales de la póliza de seguro de automóvil (minimo de responsabilidad de $100K/$300K)
